To complete your Milken Scholars Program application, submit the entire package described below. The online application is accessed via SurveyMonkey using the username and password provided.
1) Milken Scholars Application
- Complete the online application (accessed through SurveyMonkey). The form asks for Personal Information, Family Information, Education, and Activities. If you have taken AP exams, you will be prompted to upload an AP Score Report available from AP Central.
2) Two Letters of Recommendation
- For confidentiality, the preferred way to receive recommendations is to invite your Recommenders to upload their letters directly through the SurveyMonkey portal. As alternatives, Recommenders may give their letters to you to upload with your application, or they may email letters directly to the Program. Instructions for requesting and submitting recommendations will be provided when you begin the application.
3) Essay Response
- Complete and submit your essay online with the rest of your application. Be sure to proofread the statement and correct any errors before clicking “Final Submit.”
4) Family Financial Profile
- This form must be completed by, or together with, your parent(s) or guardian(s) and uploaded with your application.
5) Current High School Transcript (PDF)
- Upload a PDF copy of your transcript. It does not need to be an official copy, but it must show your name and your school’s name, list grades from 9th–11th grade, indicate 12th-grade enrollment, and be clearly legible.
6) Test Score Reports (PDF)
- Upload PDFs of your official test score reports. The College Board and ACT websites provide downloadable score reports you can use. Upload a report for each test sitting. When multiple sittings are submitted, we will superscore — selecting the strongest section scores from your submitted sittings.
2025 Selection Cycle — Test-Flexible Requirement
The 2025 Selection Cycle is test-flexible. Applicants must submit at least one of the following (and, because the Program emphasizes scholarship, additional academic evidence is encouraged):
- SAT superscore of at least 1350
- ACT composite superscore of at least 29
- AP exam(s) with scores of 3 or higher
- On track to earn the IB Diploma
- On track to complete an Associate degree by high school graduation
